目的 探讨三维超声诊断胎儿半椎体畸形的价值。方法 回顾性分析2012年1月至2016年8月在北京妇产医院经超声诊断的48例胎儿半椎体的病例,包括我院超声筛查诊断的10例和外院来我院会诊的38例,并将三维超声图像与产前或产后影像学检查进行比较。结果 在我院筛查病例中胎儿半椎体检出率为0.13‰。48例病例中,单发34例(颈椎l例、胸椎18例、腰椎13例、骶椎2例),多发14例(胸椎3例、腰椎3例、胸椎合并腰椎6例、腰椎合并骶椎1例、颈椎合并胸椎和腰椎1例);合并其他畸形16例,无合并畸形32例;足月分娩14例,引产34例。三维超声检查均可见脊柱排列异常,椎体形态改变,与产前或产后影像学检查比较,诊断准确率为91.67%(44/48)。结论 三维超声对胎儿半椎体的诊断更准确、直观,是二维超声很好的补充,能为临床诊断、鉴别及预后的评估提供更可靠的依据。
Objective To investigate the value of three dimensional(3D) ultrasound to diagnose the hemivertebrae. Methods Retrospectively analyzed 48 cases of hemivertebrae diagnosed in Beijing Obstetrics and Gynecology Hospital from January 2012 to August 2016, including 10 cases screened in our hospital and 38 cases transferred to our hospital for the further consultation. Compared the 3D ultrasonographic images with the antenatal MRI or postnatal images. Results The detection rate of hemivertebrae in the screening cases of our hospital is 0.13‰. Among 48 cases of hemivertebrae, 34 solitary(cervical 1 case, thoracic 18 cases, lumbar 13 cases, sacral 2 cases) and 14 multiple(thoracic 3 cases, lumbar 3 cases, thoracolumbar 6 cases, lumbosacral 1 case, cervicothoracolumbar 1 case);16 cases were associated with other anomalies while 32 cases isolated. 3D ultrasound demonstrated all the abberant veterbral alignment, abnormal shape of the vertebrae, compared with the antenatal MRI or the postnatal images, the diagnostic accuracy of 3D ultrasound is 91.67%(44/48).Conclusions 3D ultrasound is more accurate and straightforward method for the diagnosis of hemivertebrae and useful complement for the 2D ultrasound, providing reliable evidence for the clinical diagnosis, differentiation and prognosis evaluation of hemivertebrae.
