目的 论使用MRI测量胎儿肺体积在产前评估先天性膈疝胎儿预后中的价值。方法 用MRI分别测量病例组(先天性膈疝胎儿14例)和对照组(正常胎儿32例)的肺体积(TFLV)大小。根据测得的正常胎儿肺体积大小得到回归方程,并计算先天性膈疝胎儿的实际肺体积占预测肺体积的百分比(o/e TFLV),分析其与先天性膈疝胎儿预后之间的关系。结果 活的先天性膈疝胎儿与死亡者相比有更高的TFLV和o/e TFLV,二者差异有统计学意义(P=0.026和P=0.025);o/e TFLVⅠ≥35%的先天性膈疝胎儿预后好于o/e TFLV <35%者,且差异有统计学意义(P=0.031)。结论 用MRI测量先天性膈疝胎儿肺体积大小、计算o/e TFLV值,对于产前评估胎儿肺发育并判断其预后有较大的作用。
Objective To discuss the value of using magnetic resonance imaging(MRI) measurement of fetal lung volume in prenatal prognosis of congenital diaphragmatic hernia(CDH). Method The total fetal lung volume (TFLV) of fourteen CDH cases and thirty-two normal cases were measured by magnetic resonance imaging (MRI).We got the regression formula based on the data of normal total fetal lung volume. We used the formula to estimate TFLV of CDH cases basing on gestational age ,calculated o/e TFLV and discussed the relationship between o/e TFLV and the survival of CDH. Results The survival CDH fetuses had higher value of TFLV (P=0.026)and o/e TFLV(P=0.025) than those were not survival CDH fetuses, which had statistical significance. The prognosis of CDH fetuses whose o/e TFLV≥35% was better than those o/e TFLV<35%(P=0.031)and the result had statistical significance. Conclusions The using of MRI measurement of TFLV and o/e TFLV of CDH fetuses had a high value of prenatal prognosis fetal lung development and survival of CDH fetuses.
